TITLE: Community Navigator

LOCATION: Seattle, WA

ABOUT US:
Weld Seattle is a nonprofit organization that provides housing, employment, and resources conducive to recovery and successful reintegration back into society. We simply believe all people should be given the opportunity to belong and thrive. Weld provides transitional housing and support for community members returning home from prison, and those in substance abuse recovery. We aim to reduce recidivism and relapse by creating and engaging in authentic communities of healing and accountability.

POSITION:
The Community Navigator will act as a case management provider offering transitional support to men and women who are returning from incarceration and / or in active recovery from substance abuse addiction. The Program Coordinator will act as a liaison to and develop partnerships with various resource networks to ensure that the basic needs of members are addressed and to help facilitate a successful reentry. Strong advocacy skills are required as this individual will at times be responsible for bridging the communication gap between members and other entities, including employment partners, the judicial system, and housing providers.

Good time management is also a key component for success in this position. The case management load will include 15-25 members and appointments must be regularly scheduled and utilized productively. Accurate documentation is critical for this position and notes will be used to monitor member progress and to address priorities, challenges, and goals. The ideal candidate will be culturally competent and can recognize and respect the different nuances of various cultures and ethnicities. This full-time position will report directly to the Executive Director.

KEY DUTIES & RESPONSIBILITIES:

Direct Case Management

  • Screening and intake to determine members’ needs (permanent housing, employment, education, mental health, substance abuse treatment, legal, reunification, etc.)

  • Placement of new members within Weld-provided transitional housing network. Ensure compatibility and support with other community members and house managers

  • Provide members with support services and/or make referrals or linkages to other community agencies to resolve barriers to permanent housing, employment, and other basic needs

  • Assist members in developing measurable and achievable reentry action planning, while documenting progress and referrals along the way to completion

  • Coordinate and track collection of monthly member dues with timely precision

  • Assist member residents with move-in/out as needed, including early eviction from household when necessary

  • Facilitate educational life-skill classes, workshops, presentations with member community

  • Collect member statistics in a timely and accurate manner. Document service provided and track member progress

  • Work closely with house managers on building safe and healthy communities of reintegration where member needs are met, mutual support is provided, and goals are pursued

Member and Agency Outreach

  • Assist with outreach to individuals, institutions, prisons, and partner agencies; screening, determining eligibility, enrolling and orienting of new members and partners

  • Make presentations to community-based organizations and DOC facilities as needed

  • Coordinate and network with employers, private housing providers, treatment centers to develop and maintain appropriate referral services

Organization Operations

  • Assist with front desk work as needed, helping to answer phone calls and greeting guests

  • Coordinate bill payment to various service providers

  • Participate in scheduled meetings with Weld team members and Board, providing regular updates to Weld leadership

  • Assist with budget creation for program services and operations

  • Help maintain complete and accurate files which demonstrate Weld’s active pursuit of its mission

  • Assure that members, staff, volunteers and community partners are treated with respect and dignity regardless of race, ethnic background, gender or socioeconomic background

  • Other duties as assigned, with occasional evenings or weekends as required

QUALIFICATIONS & CORE COMPETENCIES

  • Degree - or equivalent work experience - in Human Services, Social Work, or related track preferred

  • Ability to organize, prioritize, and perform multiple routine and non-routine tasks within a timely manner

  • Commitment to diversity, including sensitivity to the needs of residents and staff from diverse cultural and economic backgrounds

  • Experience in restorative justice, post-prison successful reintegration, substance abuse treatment and recovery preferred

  • Demonstrated ability to maintain confidential and sensitive information

  • Ability and willingness to work independently and as part of a team; able to make sound judgements without immediate supervision

  • Strong oral and written communication skills with people and organizations from varying backgrounds and life experience

  • Commitment to Weld’s mission and to the transformation of lives through authentic community and supportive services

  • Act with honesty and integrity in both your words and actions

  • Ability to pass a pre-employment drug screening
